Chosen Beginnings

A Maternal & Infant Health Initiative of Chosen Women Empower

Chosen Beginnings is a comprehensive, no-cost maternal and infant health program dedicated to improving pregnancy, birth, and postpartum outcomes for Black and Hispanic mothers in rural Bastrop County. Our mission is to reduce maternal health disparities by providing culturally responsive, community-based support that empowers mothers, strengthens families, and gives every baby the healthiest possible start in life.

From pregnancy through 12 months postpartum, our multidisciplinary team—including a Registered Nurse, Certified Doula, and Community Health Workers—walks alongside each mother, ensuring she has the education, advocacy, resources, and support needed to navigate her maternal journey with confidence.

Services Include

​

Personalized Care Coordination

​Every mother receives individualized support and guidance to help navigate healthcare services, community resources, and her unique pregnancy and postpartum needs.

​

Registered Nurse Support

​Our Registered Nurse provides health education, care coordination, wellness guidance, and works alongside each mother to help promote healthy pregnancies and positive birth outcomes.

​

Certified Doula Services

​Our certified doulas provide emotional, physical, and informational support throughout pregnancy, labor preparation, birth, and up to 12 months postpartum. Mothers receive compassionate advocacy and continuous encouragement throughout their maternal journey.

​

Community Health Worker Support

​Our Community Health Workers help mothers overcome barriers to care by connecting them with healthcare providers, social services, community resources, and ongoing support that addresses the social determinants of health.

​

Telemonitoring for High-Risk Pregnancies

​Eligible high-risk mothers receive a maternal wellness monitoring kit that may include a blood pressure cuff, glucometer, Fitbit, and other monitoring devices. These tools support real-time health tracking and help mothers and their healthcare providers monitor important health indicators throughout pregnancy.

Transportation Assistance

​We provide free transportation assistance to and from prenatal and postpartum medical appointments, pharmacies, and grocery stores to help eliminate transportation as a barrier to receiving quality healthcare.

​

Grocery & Nutrition Support

​Recognizing the vital role nutrition plays in maternal and infant health, eligible mothers receive grocery assistance to help promote healthy pregnancies and improve family well-being.

​

Prenatal & Postpartum Education

​Mothers receive evidence-informed education covering pregnancy, childbirth preparation, breastfeeding, newborn care, postpartum recovery, maternal wellness, and infant development.

Health Advocacy

​Our team empowers mothers to become active participants in their healthcare by helping them understand their options, ask informed questions, and advocate for themselves throughout pregnancy and beyond.

Culturally Responsive Care

​We provide compassionate, respectful, and culturally responsive care that honors each mother's lived experiences while helping build trust between families and healthcare providers.

​

Community Resource Navigation

​Families receive assistance connecting with healthcare providers,  behavioral health services, nutrition programs, parenting resources, housing assistance, and other community support services as needed.

Chosen Women Empower Tribe

​Every mother becomes part of the Chosen Women Empower Tribe—a supportive community that provides encouragement, education, peer connection, and ongoing support throughout pregnancy and the first year after birth.
